Follow the instructions below if you were issued a passport that had a validty that is shorter than the full validity. Most of these will be if you lost your passport while overseas or did not have a certified birth certificate when you applied for your passport.
Note: Some passports are stamped inside saying they may not be extended. Follow new passport procedures if this is the case.

Extend Validity

In order to apply and receive a United States passport you must follow the instructions as outlined below. To make the process easier to complete we suggest you print this page and follow the steps in the order presented.

Letter of Authorization

Please print 2 copies and sign both with original signature. Our office must instruct you which letter to use based on where your passport will be processed. Call us below in Step 8 to determine this.

Chicago Letter of Authorization

Proof of Citizenship - You must provide ONE of the following:

Previous U.S. Passport more than 15 years old
Certified Birth Certificate issued by state, county or city (if born in US)
Certificate of Citizenship or FS-240, DS-1350 (if born outside the US)
Certificate of Naturalization

Proof of Identity

Valid driver’s license w/ photograph issued more than 6 months ago. If you do not have a driver’s license or a driver’s license that meets the requirements above then you must supply personal documents.

Letter Explaining Reason for Extension

Provide a detailed letter explaining the need for extension and the reason it was a limited passport.

Two Passport Photographs: Must be taken by a professional passport photographer.

Fees: Government and U.S. Vital Document fees can be paid by money order or credit card.

Government fees. You will write a check to “US State Department” for $60.00
U.S. Vital Documents fee depends on the processing time you choose please see the order form for exact fees.
