Passports can only be renewed if ALL 3 of these conditions are met:

1.  previous passport was issued when you were age 16 or over, AND
2.  the passport is still in your possession and undamaged, AND
3.  the passport was issued less than 15 years ago

In order to apply and receive a United States passport you must follow the instructions as outlined below. To make the process easier to complete we suggest you print this page and follow the steps in the order presented.

Your Most Recent Passport -

If the passport is still valid, make sure that it is in the last year of its validity, and if it is already expired then make sure that the expiration date is within the past five years and that you were at least 16 when it was issued.

DSP-82 Form – Print , fill out and sign. Please use the online application if possible.

Two Passport Photographs: Must be taken by a professional passport photographer.

Original Legal Records Documenting Name Change ( if change ocurred )

• Original certified Marriage license
  
• Original certified Divorce Decree
• Photocopies of documents are not accepted by the Passport Agency
  

Evidence of Immediate Travel – Must provide a copy of one of the following:

• Round trip ticket indicating travel to a foreign country.
• Travel itinerary
• Business traveler’s may submit a business letter if the above is not available.

Fees : May be paid by money order or credit card.

• $135 Government fees. You will write a check to “US State Department” for $135.00 (please write your date of birth on the memo line of the check).
• The U.S. Vital Documents fee depends on the processing time you choose please see the order form below for the exact fees.

Do not send your information to any of the addresses listed on the order form without prior confirmation by our office. We will instruct you in Step 8 (above) where to send your application when you call our office.
Same day passports are required to be sent FedEx "First Overnight" which is delivered by 8:30am.  FedEx Priority shipments will be submitted the following day.
